Hepatitis B patient referrals

GPs can refer hepatitis B patients to us via phonepost, fax or email (we have compliant MoH IT security and firewalls), or via CareSelect, HealthLink (our EDI is NZhepfnd), Best Practice or Medtech Outbox.

You can also fill in our online referral form (or get the patient to fill in themselves):

Or download a PDF: HFNZ GP Referral and Enrolment Form (195.78 KB)

What information should I pass on when referring a patient?

The patient must be a New Zealand resident or be eligible for free healthcare in NZ. If they are resident on Healthlink, please provide proof of residency or eligibility of health care. We can accept referrals for non-resident or immigration patients but please advise them they may have to pay for other health services if they don't meet eligibility requirements for free care.

If they are under hospital care for hepatitis B, please include ultrasound reports and clinic letters.

What happens after I have sent a referral?

We’ll send you an acknowledgement letter will be sent to you advising we have received the referral. A welcome pack will be sent to your patient, including a result letter, hepatitis B pamphlets and a consent form, if not already signed.

One of our community nurses will contact the patient for education and, if required, testing of family members.
